Blackberry says "Sim not Provisioned:2" what's that mean? what do i do?

i've had this sim card for almost 2 years, and my old phone died, so i used a phone borrowed from my step-mom, and then that one stopped working after a few months randomly so i borrowed another phone last week (blackberry) and it had been working fine. Then out of nowhere last night it stopped working and had that message. Any ideas what to do? i've taken the battery out, i've taken the sim card out and done this several times. Help?
