Can I get an estimate?

I went in to the doctor and my insurance was supposedly used to pay some of the cost. I asked before my second appointment how much it'd cost, and she told me 40. When I went for my second appointment, she charged me a little more than double the cost she had previously told me. I asked for an estimate, and I meant that I wanted one in writing so I know where all the cost is going to. She verbally told me the cost. I asked if it'd be more than that. She said probably...is that a good reason? I usually know the correct cost before my appointment, so why was I charged more, and why wasn't I given a written estimate? It bothers me because it took most of my balance today! What if I didn't have the exact cost ready to pay?

I didn't know how to be assertive without being rude. Can you help me with how to get an accurate estimate in writing? When they say estimate doesn't that mean it has to be that exact cost? I have never been charged more than what I have been told before, so this is new to me. Thank you.
