Can you backup an app's highscore from one iPhone/iTouch to another one?

Hi I have a 2nd gen iPod Touch with numerous games and highscores. Since I'm thinking about switching to a newer model iTouch or iPhone, I'm wondering whether it's possible to transfer the highscores with the app to the new iTouch/iPhone?
