Citroen Relay 2.2HDi loss of power/engine light on?

Have a 2.2hdi 2004 relay van. Van is fully serviced by local garage. Has developed a fault where it will either cut out or go into limp home mode when reved under load. When the engine is cold there is no issue, once its warm will happen continuously. Restarts after a few seconds if it has cut out.

So far have had...........

Fault codes downloaded - 191 & 404 - egr valve & low fuel pressure.
Have checked low pressure fuel system - Okay
Have had diesel pump checked - Okay, but needed recalibrated
New fuel filter fitted
MAF cleaned out with contact cleaner (new one ordered anyway)
ECU values checked - no issues found (voltages, earths, etc)

Have had someone suggest checking the pressure rail sensor?

Anyone agree? Other ideas?

imho i had a cataltyic converter fitted to my peugoet 206 thought it was fixed but it wasnt as needed the manfacturers make to fix it as i went down the cheap route and got a cheap one,but it needs the peugeot one for the car to cut a long story short maybe you need genuine parts for your van only the make of the van .
