The first time I saw such a person, I didn't see the headset and thought...uhm...jeez, there's something wrong with that guy - he's talking to himself.
Really, if the phone is in their pocket or purse, and you can't see the headset well, they do look possessed or mentally ill having a conversation with themselves.

The expression is 'under the bridge'
or: 'in a cave'.
Buck Nasty - lol, no hard feelings, and if you ever catch me using one you can beat the stuffing out of me.
Though I must correct and say that certainly not *every* wireless technology is bluetooth - that's restricted to mainly mobile devices, and not even all of them.
Laptops and the iPhone don't utilize Bluetooth, but instead WiFi (802.11b, g, etc.)
