Help with problems potty training puppy?

I have a 10 week old female doberman puppy who we have had since she was 6 weeks old. (Yes, I know you're supposed to get them after they are 8 weeks old, but these were special circumstances.) She is very smart and an absolutely wonderful puppy. She is growing quickly and we love her to death. She learned sit, shake, down, and high-five in five minutes each so I know she isn't dumb! However, we cannot get her house trained. She is still going in the house... a lot.

She is crate trained and can hold her bladder all night from about 11pm to 8-9 am with no problem. When we can't watch her, she goes in her playpen (5ftx6ft) and it has a bed, her food bowls, and toys in it. She never potty's in there. I know because of these things that she CAN hold her pee, but when she's out roaming the house (she's only allowed in the living room and kitchen) she doesn't seem to hold it. She just drops and pees.

We feed her 3 times a day and water her at feeding times, and then once in between feeding times, but never after 9pm. We take her out to potty as soon as she gets up, after meals, and then whenever she is sniffing around (if we catch her). I try to take her out every hour. She gets verbal praise and treats when she goes outside. She always goes really quickly. I say, "Go Potty" and she immediately goes and does her business. We're not outside five minutes and she's already done #1 and #2. I've caught her peeing inside before and said, "NO! Bad potty!" and then I immediately take her outside and praise her, but she still keeps going inside. It's like she just doesn't get it. The vet said her bladder is fine. I don't know what else to do, or why she keeps going inside. Any help would be greatly appreciated. I'm getting really tired of cleaning up puddles and piles.
