How do I link to google maps?

I'm trying to link a google map that consists of half a dozen locations to a document I'm working on. The only problem is that the url for the map is simply
http://maps.google.com/ no matter how many things I add on.

I've linked it before, but I can't remember how to do it.

Please help!
