many problems with my puppy... :( please helpp?

alright,i got a puppy about 2 weeks ago.she is potty trained,fixed,shots
all that good stuff:)but anyways,we have to put her in a crate for about 7-8 hours cause of school, right when i get up in the morning, at about 6,i give her food and water, she eats it all, and i take her out
to potty from the night befores dinner and stuff, shes maybe
done "her business" bout 2 times outside since we got her.
but anyway as SOON as i get home until about 2-4 am
in the morning, im watching her like a hawk to make sure she isnt
gonna potty/poop on the floor, but if i go to the bathroom and leave her in my room for like 1 minute, she always ends up pottying or pooping whenever im not around. she was raised on a doggy door ever since she was born and we cant get one=/ so i need help on that...
also,she is so scared of EVERYONE. if we walk close to her she
ducks down like we're gonna hit her or something and i would never
ever ever do that to any animals.no one in my house would so we
had a thought that MAYBE just maybe that her old owner hit her
a lot? because two weeks is a pretty good time to have gotten
used to us all by now, no one has been mean to her or anything so
im looking for ways to try to get her to trust me more i guess you can say? i normally just go up to her and after she'll stop running away,
ill just pet her for a while and then she'll run away? also, whats weird is she will run around the house a whole lot to try
to keep her pee in or something? i dont know, im just really tired all
the time and starting to do bad in school from staying up to late
at night, ever since we've gotten her, ive probably gotten bout four to two hours of sleep every night for two weeks and 3 days im pretty sure.. and im still not gonna give up on her so i have to stay up cuz i want to a good mom for her but im exhausted....so i would REALLY appreciate it if anyone at all could give me some help.
