My dog waits for permission to do everything?

She's 4 years old and I adopted her a year and a half ago and have been training her ever since. I've always taught her to wait for everyone to go through a doorway before she's allowed to go, she needs to wait for someone to say it's okay to eat her breakfast/dinner and she's not allowed on couches or beds unless someone says it's okay. But I think it's getting slightly ridiculous.

If I'm in the kitchen, she'll sit in the doorway and stare at me until I tell her she can come it. She'll sit in the doorway of the back door, even if it's freezing cold and raining, until she gets permission to come inside. If I go outside, I have to tell her to come out with me and I need to encourage her sometimes because if I make a move while she's going out the door, she'll back off and wait for me to tell her again.

How can I let her know it's okay to do somethings without permission? She's not one of those dogs that have the personality trained out of them, she has more attitude than any dog I know, it just seems like she's afraid of getting into trouble sometimes. I've never hit her, but she was a pound dog with 3 previous homes, so anything could have happened to her then.
