Nutrition graduate, onto OB/GYN?

I have always been very interested in nutrition, as well as maternal and child health. I am getting closer to graduating with my nutrition degree, and it is coming time for me to decide if I want to pursue becoming a registered dietician (which involves being accepted into a highly competitive program at my school, and going on to take the RD exam) or just graduate as a nutritionist.

On one hand, I don't want to limit my career opportunities by NOT becoming registered as a dietician, however, the coordinated program (as it is called) is highly competitive and honestly scares me to think that I may not even be accepted - in that case, I would have to take a year or so extra doing an internship on my own, risking beginning my career with no salary.

I have recently been thinking about using my nutrition degree and going on to studying obstetrics. Of course I understand that is also very competitive, but the area is very interesting to me. Does this route even make any sense? What would you suggest?
