vibrating and shaking on 99 ford e350 7.3 diesel?

i have a 99 ford e350 van with a 7.3 diesel. it has a little over 200,000 miles on it and it shakes and vibrates the entire van at idle, give it a little gas and it smooths out. dirty fuel injector maybe? it is a one owner, bought new so it has always been maintained, so what could be wrong with it?
