What do asylum records/papers look like?

I'm writing out the records/papers (or whatever you want to call them) of a character of mine who is schizophrenic. I have all of his information (obviously), but I'm pretty sure I'm organizing his papers in the asylum he's admitted to wrong.
Does anyone know where I can find a sample or something of asylum records? One that I could use to fill out my character's information to his asylum?
