what is the PUK code for a rogers phone?

ok, i just locked or did somethingg with my sim card on my phone. so i cant call ot text. all i need is a PUK code to get toall my texting and calling. i tryed the last 4 digets of my number, *411, * (4 digets of my #) and i dont know what 2 do, may i have help?
