What's up with FIFA 12?

Am I the only one who's had this issue? When I play manager mode the game seems to go buggy, as in when I save, quit and then come back another time, often my team is now a bunch of goalkeepers and my custom formations have been deleted. Also I've found that it occassionally freezes and a player won't accept a contract when trying to sign him becuase he says that his club would want more cash, even though they've already accepted the offer? Another issue has been Ultimate Team, especially the web app. I've found recently that it does not recognise me having an online squad registered on any platform, even though I can perfectly well play UT on my PS3.

If you know anything about any of these problems then please let me know, it's driving me insane!!
