Where can I go with this premise?

I have an idea for a story, but I don't know what to do with it. Basically these two brothers grow up their whole lives thinking they're identical twins. But later, they find out somehow that they are not twins - one of them was actually cloned from the other during infancy because he was sick and the doctors didn't think he would make it.

I'm not sure where to go from there, though. I mean, there's so many different directions I could take this. If you were writing this story, where would you take it?

Maybe explore the concept of personhood. Is a clone a real human with a soul and feelings?
Or maybe create a clone oversight agency that tries to terminate the clone because the original guy survived.

Also, going off of what Alycia said, ask the original how he feels about being cloned, and how the clone feels about being a clone.

Keep asking yourself questions, it's the best way to open up the story? Why would the doctors need to clone him? It's been done before to gain body parts? But why? What's there true backstory? Ask the characters, and ask yourself. I would research it and see what path you like best. What interests you? Looks up a basic idea of cloning and reasons behind transplants and other issues!
