why did my dog poop on my bed/pillow?

My boyfriend and I adopted an 8 m.o. puppy about 2 months ago. he has some behavioral issues. He is extremely hyper and has seperation anxiety--if we leave him too long (4-5 hours), he will tear up the house.

Since we adopted him he has always known to poop/pee outside and is very good at holding it. He has made mistakes before, but it's always been our fault...and he just goes right there on the floor by the door.

But tonight, out of nowhere, we left him for about 2 hours longer than usual (accidentally). When we got home we discovered that, surprisingly, he had NO seperation anxiety. But he pooped and peed on our bed (!) right next to our pillows. He has never done this before. My boyfriend is very upset and I am just worried. I know he is just a dog...dogs make mistakes. But he usually sleeps with us (head on the pillow :P) and seems to just know better than this.

I have heard that when dogs/cats poop near your pillow or in your shoe it is a sign of being upset/frustrated. I highly doubt this but am curious.

Was this just a random mistake or is this a sign of some other underlying issue?
