Car shakes when I slow to a stop, not putting gas into it, or when I am climbing uphill and the car shifts to a higher gear, the rpms drop and it the car shakes, and loses power, but if I manually shift it to a lower gear it works fine, any help?

I have a 93 Nissan Altima, the car started shaking a week ago, whenever it is running and back up or stop for red light, it won't shake when it is in park position. I changed the air filter, the distributor cap and rotor, but it is still shaking. What is the next step I should do.

I work out 4 days a week and drink a protein shake after I work out. I get a nasty case of acid reflux/heartburn after I consume the shake.
Does anyone know what causes this or anything to prevent it?
