vibrating

It shakes when it's idling, is sluggish, and when going up a hill it jerks feels kinda like I'm driving over speed bumps or something. I'll start up the hill and since there's very little power the rpms drop quickly and it starts jerking. Now there are spurts when it's cold where it will run...

causing this.? It gets louder and the car vibrates more when i turn left. When i turn right, the sound and vibration goes away. ANY IDEA WHAT MAY BE CAUSING THIS?? any help much appreciated. I drive a 2002 Toyota Sienna. thanks in advance!
