What is the proper way to shift into and out of 4WD? (including low and high)

You can shift between 2h and 4h while driving, it is best at lower speeds like 30mph.
As far as N and 4 low the vehicle must be stopped with the transmission shifter in neutral, sometimes you may need to shut the engine off because 4 low is not synchronized.
