Are CD/DVD lens cleaners really effective?

The warning always say NEVER TOUCH THE LENS. Otherwise, the lens will be damaged. So i think a cd lens cleaner is just a hoax, cuz if those rotating brushes really touch the lens during cleaning process then it should've created fine scratches on the lens resulting to damage.

I'm not sure. Its just my theory. Anyway, what say you? And what's the best and safest say to clean the lens?
