Who killed the kids? And who was actually Mildred Kemp? Was that Anne too? God that was a freaky movie but what a great twist.

Mildred Kemp killed the kids. Mildred Kemp was the lady with the blonde hair in the mental instution, it wasn't Rachel Summers though. Anne didn't kill the children, but Anne killed Rachel Summers and caused the fire by accident. Rachel Summers wasn't evil she just acted that way because she had an abusive father and changed her name so that her abusive father wouldn't find her. Anne made the whole thing up because she was a psycho.
