help with a fitness log?

Ok, I'm still studying English and I'm taking PE classes in college. I found a fitness log:

What should I write in 'reps' and 'set'? I don't get what those mean.

Also, I'm using these:

In weights, what should I put? Weights 15LB?

reps or repetitions means how many you do in one set. you can have 2 sets of 10 reps for pushups which means you do 10 then you rest a little and do it a second time.

a set is a single amount of repetitions you do until resting. a rep (Repetition) is how many times you perform the exercise in each SET. So, if you pick up 15 lb dumbbells (The picture you listed) and do 15 curls, that is 1 set of 15 reps. Usually you do more than 1 set and the repetitions can change, it all depends on what youre trying to achieve from the workouts.
