How can I get Tibet travel permit for traveling from Nepal to Tibet?

Bangladeshi national wanting to go to Tibet via Kathmandu from Dhaka, Bangladesh. Need information regarding how to get it & where? Do I need the Chinese Visa as well, or is it possible to get that from Kathmandu if required?
