How do UFC staticians/referees/scorekeepers tell the difference between a TKO...

...and a knockout? Definitions:
TKO (technical knockout) when a fighter is not intelligently defending himself and the referee stops the fight. It can also occur when the ref/doctor stops the fight.

KO (Knockout)- When a fighter loses conciousness or is not able to immediately continue after a legal attack.

How do the staticians tell the difference? A lot of times immediately after the fight, it is announced as a KO, but then changed to a TKO or vice versa. Such as today, I think I remembered Bruce Buffer annoucing that Paulo Thiago's victory over Koscheick was a knockout, but on the UFC website it said a TKO. No doubt Dan Hardy's win today over Rory Markham was a KO.

PS: How come Josh Koscheick stopped using the nickname "Kos"?
