For the coolant, the sensor is probably corroded or bad. That's not to hard to replace. As for the shifts, if it is automatic, it is probably the clutch packs in the transmission. When they get warn they might skip or be jerky when shifting. Have the trans fluid changed an if that does not fix it, have the transmission inspected. If it is manual, it is probably one of the synchronizing gears.
