Is my workout routine well thought and how could it be improved?

Recently, for the first time in memory, I've started a workout routine in addition to watching my diet. To get to the point, here are my statistics:
Age: 17
Weight: 300
Height: 6'0" (maybe an inch taller, shirts are seeming too short lately)
Waist: 44 inches
Other: I've been overweight since 3rd grade (7-8 years) and I've done little about it. Both my parents and older sisters have always smoked for as long as I remember, so I'm forced to live with 4 smokers. My calves are virtually fat-free, while my thighs contrast with fat.

My workout routine which I've been incredibly loyal to, hitting about 4 times a week for 3-4 weeks now is as follows.
Punching Bag for about 5 minutes
Dumbbell 15 lb lifts (60 curls, 60 side-lifts, 75-90 shoulder presses for each arm)
Punching Bag for another 5 minutes
Same Dumbbell routine
Punching bag for another 5 minutes
Bar weight, 60 lbs, 30 curls, 30 squats, 30 shoulder presses
Same Dumbbell routine.

My diet consists of an oatmeal breakfast with orange juice and coffee (only plain cocoa added,) lunch consisting of a chicken patty sandwich and a water bottle (I'm still in school, not many choices,) whatever my mother makes for dinner (usually balanced well, ALWAYS with a tall glass of milk) and if i do snack, which i try to resist, i make it small and try to make it as healthy as possible.

My question as stated above is, how well thought out is my workout routine (when it gets warmer i intend to try and jog some too) and how could it be improved?
Also, could my sudden shortening in shirt lengths be from a growth spurt, more specifically from working out?
