Massey Ferguson tractor tire? Why did it come off the rim?

I have a Massey Ferguston tractor (well, my husband does) and we've had it since March of 08. The front tire came off the rim tonight while he was plowing (2 inches of slushy snow on our paved driveway). WHY did this happen? Could low tire pressure cause this? Would a bad bead cause this?

Thanks for any info -we just want to prevent a problem in the future.
