My 2005 Nissan Altima shakes when I start the engine?

Im not sure what it is that makes it shake for minutes when I turn it on; the originating problem was when on turnpikes, the car would shake uncontrollably when I would stop at a toll unless I put the car in neutral, it felt as if the gears were out of place or something [car is Altima 2.5S] this only happened when I would be going at a high speed for long periods of time and then I would slow down to a toll or exit.. it then progressed to where I am now and it feels when I am at low speeds, as I put my foot on the pedal, it is vibrating
What could this be?
