...PUK code is PLEASE HELP? i was looking at the phone trying to set a lock on the phone and that PUK code just popped up.
 
If you have AT&T you have to take it to the store and they'll fix it for you.
 
take it to a repair center and they'll reset it for you.
 
If this an AT&T phone you can get the PUK code from your online account or by contacting customer service. You only have 10 attempts to enter in the PUK code. If you enter in the wrong code too many times you will need a new SIM card.
