My health assessment says I have poor nutrition but I don't understand...?

I took an annual health assessment for my insurance company and it indicated that I have poor nutrition. I indicated that I eat the following servings:
3 servings of fruit/veggies
4 servings of whole grains
2 servings of protein
1 serving of dairy

Now, the dairy I understand. Ideally I would eat more, but I don't like milk so I take calcium supplements as requested by my OBGYN. I don't understand the rest. According to the results, I'm supposed to have 5 servings of fruit and veggies, 4+ servings of grains and 3 of protein. I am a small person. I cannot possibly eat that much! I eat when I'm hungry and until I am full but I don't see how I can possibly eat any more. I do take vitamins to ensure appropriate nutrition. Am I really as high risk as this assessment says?
I'm not on a vegetarian diet. I have a turkey sandwich or something like that for lunch and for dinner I have either chicken or beef or fish. I'm getting plenty of variety, the results just say I should eat more food in general. The problem is, no more food will fit!
Gary I think you should read the question that I posted. Not only would you see that I said nothing about being vegetarian but you would see that I said nothing about the types of supplements I take so you could make no conclusions that I am taking or not taking the correct ones.
 
Wow..LOL..I would SO Fail that test! Don't worry about the results of a silly test! What do you expect from an insurance company. You eat more than I do! To me, your doing fine. Some people don't even get half that! Don't worry about it.
