my puppy wont potty train fully and hates his crate?

We got our nine week old puppy a week ago. we brought him home and he was great. we expected accidents cuz of the new surroundings. but each day gets worse and worse. We take him outside and wait 5 , 10, 20 min and nothing. we take him back in and within thirty min he's peeing or pooping somewhere. and everytime i catch him in the act we take him outside and remind him this is where we go potty. Sometimes this even happens AFTER he does his business outside...he comes back in and does more business inside. He never has accidents over night and he wakes me up in the morning to let him out but thru out the day he never notifies me he has to go to the bathroom. i take him out every two hours or so just to see if he has to go. im cant even explain how positive we are and congratulatory when he goes potty outside. we do the treats and the praise. when we catch him in the act we say No and take him out immediately. We bought a crate and this is my second problem...he hates it. He cant be in it more than 3 seconds without going nuts. howling and whining and barking. we do toys and treats and the whole nine yards there too. He will whine and cry for however long we keep him in there and when we're home it is minutes maybe. We also crate him while we leave the house cuz we have cats and he is a puppy after all and chews everything.

So recap...potty training and crate training problems. I'd really appreciate some help. He's and amazingly smart puppy and follows commands so i feel like im missing something. thanks for the help!
