Potential Automatic Transmission Issues (1998 Toyota Tacoma SR5)?

Hello. I've had this truck for a little over 2 years and have had little problems with it. Changed the oil and wipers and it's been golden.

Anyways, lately I've found that when driving off, after 2nd gear or so, the engine reaches a rather high RPM before hitting the next gear. This gear seems to be the most troublesome, but also when hitting higher speeds (50+), the next set of gears will pass quickly, with little RPM increase in between.

Hopefully I've somewhat described the issue well, as I'm rather automotive retarded. The long and drawn out push to 3rd is what really worries me. While I expect the next shift in the 30s and 40s, sometimes it can reach nearly 50mph before hitting the next gear! In addition, sometimes when I let off the gas slightly and then reapply, it will occasionally coax the thing into shifting at what I'd imagine is a more suitable time.

So, the question with all this is, what should I be looking for? Or, what am I looking at (repairwise)? In addition, are there any easy measures that I could take (possibly some solution that can be poured in to clean things up [seen something similar for fuel injectors I think]).
