The bolt on training wheels for bike keeps unscrewing while child rides?

I got a training wheels set but even though I tighten the bolt and follow the directions the bolt that holds the wheels on keeps untightening and falls off.
 
Put a washer in between the the training wheel bar and the nut. If it still unscrews, get another washer and nut and double lock it. If you can find a step nut, that would be better!
