There's an old 1997 Dodge (Grand?) Caravan we may buy as a second "junky, everyday" vehicle. It does fine except, besides reverse being out, it also sometimes will downshift to 2nd or 3rd gear around 60mph. This causes the tachometer, which will normally be around 2000 rpm at this speed otherwise, to suddenly skyrocket to around 4000 rpm.

It does it, say, once a week, and it refuses to go back to overdrive unless you pull over, shut it off, and start over, then it resumes fine and stays that way, again, for a week or so.

Although reverse is out and has been for sometime, this doesn't strike me as a transmission problem, as there is no "slipping" otherwise, there's no delay in upshifting as you gain speed, no delay in engaging in "Drive" to start with. There's just the once a week or so downshifting that isn't asked for.

I do realize this is an old vehicle to start with and nothing of a beauty prize by any means, but what is the likely cause of this behavior, if not the transmission itself?
