Weight Loss Help... I need some guidance/tips/strategies PLEASE!?

I have been obese almost all of my life, but it only recently got bad because of my medication. I want to lose atleast 30 pounds first, or maybe even 15 to start but I dont know where to start.

I NEED GUIDANCE!!! Thanks guys! Really appreciate it!
