What are the digits for puk card for Motorola razor v3?

I was fittiling around with my phone and accidentaelly blocked my sim card, I read that you need a puk code to unlock the phone so what the puk code for my phone? Could anyone help me please?
 
you need to call your service provider.. DO NOT TRY TO ENTER A CODE.. YOU WILL DISABLE YOUR SIM CARD!
