What error message does the computer give you if you insert a badly

scratched CD/DVD into the drive? My CD-ROM driver won't read certain disks. When I put in the disk, it makes some of the usual noises and spins, but then goes quiet. I go to My Computer and click on the D: drive, and I get this error message, "D:\ is not accessible. The request could not be performed because of an I/O device error." I'm trying to figure out whether this is because of something faulty in the hardware/software, or because the disk is damaged. The disk does not look damaged to me (maybe a little smudge?), and I know it has worked before because it worked on my old computer. The other disk that no longer works has been used a lot, so it's more likely to have some damage.

I have a Toshiba Satellite with a Matshita DVD-RAM UJ-841S drive and Windows XP SP3. I have Googled the error message and tried some of the fixes, but they have not worked, which is why I'm wondering if this message could also mean a damaged disk.
