What is a fair system for charging customers for travel time and expenses?

In a low population area, people, such as plumbers, roofers, electricians often have to drive many miles and spend hours just to get to a work site. The job itself may take a relatively short time -for examples, installing some new wiring, or replacing a trap. While the worker is driving, he or she can't be working on other jobs. What if there are two separate jobs a few miles apart, but far from the worker's home base? What if one job is, say, 50 miles away and the next, that sane day, is 30 more miles up the road. How do you figure it so that it's fair to everyone involved?
