The key's in the psychological. You need a character with some kind of mental issue. Think of the concequence of that mental issue (it could be murder or suicide or kidnapping, any of that exciting stuff). Then you need someone trying to work out the psycho's mind, preferably with mental problems of their own. Start at the end and work backwards.
