Where is 12 digit activation Code for telus prepaid?

I can not find the activation code for my brand new telus prepaid sim card, It says the number should be on starter card its not. I bought the phone online so I can take it in! I HAVE BEEN TRYING FOREVER to get this set up and it wont work!! Some help me ASAP!
