Who introduced 'Care in the Community' and is it working?

I'm not saying that Victorian mental institutions were anyone's idea of fun but now most of them (and their grounds) have been 'developed' into luxury flats etc which must have made massive profits for the developers) what about the patients and the community? Are the patients receiving the care and support they would have got in a hospital? and is the wider community as safe as it should be?
