3 months- out of the state, how to keep car registration?

I have to go out of the state for 3 months and am going to suspend auto insurance.

1- In this case, I need to sign Affidavit of Non-Use form, isn't it?
2- Will DMV charge me agian for my renewal registration when I come back?
