fetid odour. She has discomfort in 15 whic? A 50 year old female patient comes complaining of bleeding gums and fetid odour. She has discomfort in 15 which has a crown done 7 yrs ago. A distal pocket of 5mm.
What is ur diagnosis give reasons for the same. We were given an OPG, Bite wings.
What I saw:
I could see a pocket, a post core and endo-treated which was short of the apex by 3-4mm and the crown was overhanging. I could also see a periapical radiolucency just on the mesial aspect of the root apex.
 
Lousy root canal (short endo treatment), tooth decay (pocket & odor), gum disease (bleeding & odor), poorly fitted or poorly cemented crown (allowing seepage to cause decay), lousy root canal (radiolucency on mesial of root apex). Perhaps the overhanging crown is due to decay or ill-fitted crown.
