All four paw pads fell off and nose cover came off?

My dog is experiencing symptom's that I can not find any information about on the Internet. Can anyone provide any information? following are her symptom's: all four paw pads fell off and are fleshy and inflamed, nose covering came off also fleshy and bleeding at times, under the ears skin is bleeding - I don't think blood is coming from inner ear, she has not went potty for three days - prior to that runny, having a hard time breathing out snout, shakes often as though cold, appetite good almost ravenous, eyes watery and sometimes seem glossy but other times look normal and alive, temperature seems warm, anus inflamed. vet put her one pain killers and clavamox 250mg 2x daily. She does not seem to be getting better and is in pain especially when she stands. we have been trying to keep her comfortable but i am concerned about her not going potty. all that food and drink that is going in has to come out sooner or later. what can i do? please help....
