Best calendar software/apps to link home computer, work computer, iPhone, iPad?

I'm wondering if anyone knows of a good app that will sync my desktop at home, desktop at work, iPhone and iPad all automatically?

For example if I update an appointment on my work computer it will update on all other devices.....

Any suggestions are greatly appreciated.....
