I'm an actor, and I've done sex scenes before, but they weren't real (in the sense that there wasn't penetration), and I think that's usually the case. Also, as you suggested, body doubles are sometimes used, although the ones I did were all me.
However, in the hardcore porn industry, where the actors do have sex for real, then there's a lot of STDs going around.
