I am on intermittent FMLA leave for an ongoing problem they have yet to diagnose in me. It was approved for 10 months. I went into the hospital for two weeks for several procedures. When I was ready to come back, I had to fill out a fitness for duty form to return. My employer is now telling me that I no longer have my original FMLA because the fitness for duty form was filled out and returned to them with "no restrictions". Is this accurate or do I have to get new FMLA forms filled out just because I turned in a fitness for duty form?
