How can I fix a scratched ps2 disk?

My ps2 disk is scratched and it also has a few marks on it. When I put it in the console it starts to load but then it freezes, I can turn off my ps2 when this happens though. I've tried wiping it with a disk cloth and washing it with water, which usually works. This is the first time in a while that I have played it but it has always worked. Does anyone know an easy way to fix it from home?
