How can I unlock/ unblock a envy 2 cellphone to reactivate it with u.s cellular?

I had verizon wireless before an with that an envy2 cell phone. However, due to increase in prices I moved on to U.S Cellular and I would like to use the same phone as with Verizon Wireless which is an Envy 2. How can I do this for real and efficiently?
