How hard is it to disguise someone in the real world (superhero related)?

You see superheroes wearing all sorts of masks to hide their identity. But i was wondering, what would actually work in reality? I'm pretty certain a domino mask wouldn't do shit. I mean, they can still see the rest of your face and your hair style. And if you had a beard then a half-face mask wouldn't work either, assuming it would work at all.

But is a mask even necessary? Is it possible to hide a person's identity using paint/make-up and a wig? I believe there was some show on mtv years ago which involved doing this alot.

My question actually relates to a supervillain i'm thinking up. He's a clown-themed villain, but unlike the joker (my villain is actually more like marvel's Arcade character than the joker btw) he doesnt normally look like a clown. Its all make up, and a big red wig. I was wondering if this would do anything to hide who he really is. I dont like domino masks, bc the whole idea is just stupid. The ppl in the dc and marvel universes must be so blooming stupid that superman could get away with just wearing a sign saying 'i'm NOT clark kent'.
 
A simple mask wouldn't do anything. Try Tuxedo Mask from Sailor Moon. He wore a hat and white glasses but they still couldn't tell. However, some sort of identity cover is needed in order to allow the vigilante at least a mildly normal life. If people knew who the super hero was, they would be constantly asked for help for the most mundane of tasks. People would learn that they don't have to rely on themselves when something tragic happens, because the super hero would take care of it.
